  Nice work .. !
  The fields will be long, especially the names and street addres.. feel free to have one box at each line if you think it will nice.
  Also, how about aligning the labels to the right and the fields to the left ?

     Dear, Thanks for your feedback. You want long fields for name, street address etc. But Are you know email template have limit for width. Hope have no need for longer. Because when i convert html then automatically text will be break & aligning the labels to the right and the fields to the left . see this link please: https://imgur.com/a/26Scw2x
