This contest is for making a slightly automated Excel template for me to use with sub contractors.

The fields must be

The top should have a place for our details which are:

Congo Store SARL, 355 Avenue Kilela Balanda, Lubumbashi DRC. It should also have the congo store logo(uploaded).

Company Name (these next section should be selected from a database)
Representative Name
Contact
Address
Mobile

Site This should be selected from a list
Division This should be selected from a list
contract category This should be selected from a list
Contract Description This should be a manual field (of decent size)
Contract Amount this is manually entered
Terms & Conditions These are manually entered, just leave a blank space for it
Approved By (this should be selected from a list)
Position (should be determined by above selection)
Signature
Subcontractor Signature

Employees to Work on Site (title)
Name ID NO Phone Number

should be fields for upto 10 rows

Should be a chart for contractor to manually fill out

Payment Details (title)
Date of Payment Amount Paid Signature

should be fields for upto 10 rows

again to be manually filled later on

It must include a way to add data into a database in one sheet and allow for selection of details to make things quick. example all the subcontractor details, approved by person, site etc should be from one sheet where we can populate it. It must also store all previous contracts in the excel file.

The generated contract should be set to be two a4 pages as per the instructions. at the bottom should be the four division logos (attached on the contest)

Hi, Can you confirm the format of your expected output? Do you want the excel file to generate a word or PDF document? Or do you want the output to be in the form of a printable excel sheet? Also when you say database, do you have a database already (eg sql database) that you expect the sheet to connect to or are you loading the data in one sheet and the output in another within the same excel workbook
