Design one user friendly one-html-page bootstrap checkout form. You can see a very basic example of what you are supposed to do in example-again.png attached. You should ONLY DESIGN THE ACTUAL ORDER-FORM/Checkout-functionality. Some people post entire website into this contest, but I only want the checkout/order-part of this one page. So in example-again.png, I have greyed out everything you should not do anything with.
Should include 4 dynamic "sections" using default gray bootstrap-style elements to expand. I want it about as clean as my current design. Please use my attached example.png as a template. You can see step1-step4 attached, that is my current layout.

I reccomend that you draw the design for instance in Photoshop (using bootstrap-elements) and publish your suggested design here before you do any coding (to save you time in case your design is bad).
If your design win, you must code it using HTML/bootstrap/css/js so that the different sections and buttons/forms actually appear/disappear using as much of default bootstrap only as you can - keep any custom javascript/query at a minimum. Only one color for buttons. No PHP/MySQL-coding required.
You should also adapt it to suite my current files (html/css - I have attached my style.css and part of the general html-template header/footer). So that your form will look like you designed it without using a lot of extra css-files.
NO frameworks are accepted beside bootstrap and you must mention it if you use anything else than default bootstrap-features. As few files as possible.
For ajax-login on this page: Do a dummy call to ajax-check.php to possibly du login-check and assume success for now.
It should be easy for me to generate html from PHP. For instance, I can't generate javascript as easly as html. So adding html with transport/delivery options into the html should work instead of having to generate a lot of javascript into the html if you understand what I mean. This is why I love bootstrap, you can do a lot of html and don't need to worry so much about javascript/js-code.
Description of content needed:
1. Shopping cart
Listing of:
product_name, product_size, product_num, item_price, total price.
In my country, all end-users pay tax (25%). The price excl. tax must be shown
Option: Continue or Shop more.
2. Customer information
Default show login-box (email + pass)
Option: Register (with option Private/Company),Name of user, E-mail, Phone, Street1, Street2, Zip, City, Country and checkbox to choose another/different delivery address.
If checkbox with use other/different, dynamically show Street1, Street2, Zip, City, Country again.
If checkbox company, use input box for company.
Checkbox for newsletter.
3. Review order, with option for Delivery method and Payment option. See attached current example. The review screen looks pretty OK.
4. Complete, with thank you note (and locking the unexpanded sections).
Size: Min 400px, max 750px (prefer mobile friendly/resize friendly, like bootstrap offer by default).
Note: When having form for postal nb/city, these should be on the same line.
