I have two invoices, built in MS Word, that need to be revised in such a way that everything on the invoice is protected except the fillable fields for name, address, amount, etc. The final product should be in pdf format so that I can easily email it to my agents who can fill it online and then save it to their personal computers and then send me a pdf copy. The attached files are in jpg but I also have MS Word versions which I was unable to upload for some reason as these have docx file types.

Looking for someone to create intake form from Word documents and PDFs; knowledge of HotDocs helpful but not necessary. I have an intake form which is filled in by pen. I want to make a PDF that has fields that can be filled in by keyboard, so that the data can be exported and printed. The intake form is about 12 pages and it pushes names from fields into various other fields on another 30 pages, mostly name and address, to those pages.
