Build a Login control ASP.NET -- 2

Need a login control.
Tech: ASP.NET | MVC | EntityFramework | Visual Studio | C# | SQL
If you wanna do it with traditional SQL queries, that is also OK.
Prepare a role based login control. Need to use it for a bigger application.
Admin role will be added from backed to database. Login by admin role, create new user, assign role to them.
Roles:
1. Department Head
2. Department Sub-Head
3. Assistant
4. Super User
5. Admin
Based on role, pages will be viewable. So need to handle that also. I have application prepared. Need to integrate this login control to that application. And add connotations to Views, based on roles.
Login control will have standard fields,
UserName, Password, First Name, last Name, Email, Phone.
Forgot Password
Reset Password
Update Password
Use standard ASP.NET MVC templates from visual studio, as the application is designed using them so it will be easy to integrate.
