Convert tick data into a format importable into MT4

I am looking for a simple program that can convert tick data into a format that is importable into Metatrader 4. I have got 2.6Gb tick data (from 2004 - 2011) in the format:
"datetime;bid;ask" in csv
and need to convert it into the format
" date; time; open; high; low; close"
The output file has to be able to be imported into Metatrader 4. A picture of the format in excel is attached.

I already have implemented the conversion utility "csv -> csv". Could you send me file with sample data?
