I have 2 forms (form 1 and form 2) see attached that I want to merge into 1 document and then be able to send that out to people to fill in via email. It is important the forms are easy to fill in and look nice and professional. I have attached an example of the type of document i would like it to be like as they are so nice and easy to fill in. Although i would like a copy of the form both in word and pdf if that makes sense.
