I need the attached page, [login to view URL], converted to an email form according to the graphic on the page. The designer is very picky about looks, so the form must overlay the indicated area exactly. Since this is a single page, I am looking for bids in the area of $100. The customer would like the site up quickly. I have already created the other two pages but the form is causing me problems.

Please ignore the [login to view URL] file as it is an older copy that Freelancer will not let me delete.
