Changes to Checkout

I would like some changes done on the final checkout screen of my shopping cart.

1.) The state should be a dropdown menu, instead of input, and if they select "MD" 6% tax should be added on subtotal, not shipping.

2.) When you click "same as shipping" everything copies over except for phone/email.

3.) The CC card expiration should be dropdown format.

4.) There should be a "confirmation page" that sums up everything entered to double check accuracy.
