Create A Contact/Web Form For Me

The web forms on my website are awful looking and don't invite people to contact me.
I want you to change that. I'm looking for a clean and big web form for people to fill out for me.
My website is wordpress and I would like the contact/web form to easily be copy and pasted into the end of all the pages I want the form on.
It would have the inputs:
Name *required
Email *required
Website
Subject * required
Message *required
And a captcha to prevent spam.
Right now I use a Wordpress plugin called Contact Form 7 [login to view URL] and the Really Simple Capcha plugin with it [login to view URL]
If you can create the form using these plugins, great, If not, I don't care just as long as it's easy to add into my webpages that need it.
I need two different layouts. One layout will be full width to go on a few of my pages. The lyaout of that form would have thee inputs on the left
Name *required
Email *required
Website
Subject * required
With the Message *required input on the right.
The other style would go on my blog pages and be slimmer. So all the inputs would be on vertical.
As for the color scheme, my webpages have a white background and I want to keep it that way with the background of these forms. So inside the inputs would be a little darker to make them stand out. The "Submit" button would be one of my branded colors.
I really like the wider and taller input fields along with larger buttons that is common trend in web design now a days.
I'd like to be able to add a call to action at the top. Something like "take the first step to improved search rankings" or something that I can easily change.
That should be it. Lots of details in this job because I really particular for the vision I have.
