Build a Website

I am gathering 6-12 random people. I want to design a site to gather their data. What I want is: -To give each person a unique code for them to login. Once they login, they give name, age, email, mailing address etc. - I want to capture their Internet signature so that they can sign a Confidentiality Agreement online - I want them to electronically sign another form that says they have no allergies to: food, shellfish, eggs, nuts etc. - I want them to answer 5-15 questions. Some questions would be pull down but others would allow them to write. -They would be allowed to login later, once I checked all above, to download information about my product. There would be a yes/no button for them to agree to attend the focus group session, in-person, and give them the address. After I meet with them in person, they would log back in to give feedback about what they felt about the in-person session.
