Build a Website

I would like a smart phone/tablet friendly website built to assist firefighters/EMT's with checking in their apparatus. The goal is to have them use their smart phones to ensure all equipment is on the apparatus and everything is in good working order each shift. The site should allow them to login, select the truck they are checking in, a list is then populated with the equipment that should be on that specific truck. They can use a check box to indicate the equipment is there or not. If something is broken or missing they should have the ability to type in any additional information about the issue and attach a picture if needed. After the form is complete and based on what is missing from the truck an email is automatically sent to specific people to notify them of the problem or missing equipment. This is my first time to post and I'm not too sure on how much a project like this should cost but I wouldn't think it would be a hard job for someone who knows what they are doing, most likely can copy another project and modify to meet our needs.
