Customize Genesis Enews Extended Widget

Please take a look at [login to view URL] look at the sidebar and you will see the signup form has a photo of Derek. I want that for my blog too. But my blog runs on Genesis Framework and need you to customize the Genesis Enews Extended plugin so it appears like the one above. I want to place another form below the posts too. I have the CSS and the image but the form is not Responsive. I need you to check it out too. More info will be given to the best freelancer.
