Animated offer slide down with form/banner

We wan't to implement a drop down ajax type div that can contain current offers, a signup form and more.

Here is an example of what we are looking for:

Just need the code/css developed with a generic form styled nicely. We can then implement the form functionality and add to our site on our own.

This goes under a header. I can give you more specifics once we agree on pricing.
