Adding upload functionality for uploading files

An existing website ([login to view URL]) requires a modified upload function. More specifically : The website is intended to be used for uploading and downloading study notes by its users. 1 - The user is prompted to choose a document, a university (from a complete list of all universities) and then type some keywords to describe the document (such as course number, title, professor, etc). 2 - Then a folder will be created that has the title of the course which the user provided and the note will be automatically published and placed in that folder. 3 - Also for users looking for a specific note from a course, I need a simple function to prompt the user to choose a university and a department, then all the courses (that users have added so far) will be shown and user gets to select a course to see all the notes that have been added. Note : The main framework for doing most of the above is already done and available on the website. For more step by step instruction find the attached file.
