Android App to Submit Daily Field Reports

I need an app developed that will take a standard company form (currently filled out by hand) and turn it into a simple data entry that upon completion you can send the completed form to an email address.
For example there will be drop down menus for things like job name, project manager name, date, weather details and then there would be fill in categories like employees, work performed, hours, safety violations/accidents, notes, etc.
Once the form is completed, you will just be able to hit "send" or "complete" and it will email to a predetermined email address.
Current form (in excel format) will be available as a template.
