Apple/Android App front end for Sage CRM Advert booking form.

I want an application building that is very easy to use for an advertising sales person using an Ipad or Android [login to view URL] even an end user/customer to download and complete. The data would come from Sage CRM or be imported/entered into Sage CRM . i.e. customer name and general accounts data. Ability to show previous ads in thumbnail or image/pdf format and click to enlarge. The rest of the application is basically a booking form that takes the customers order.
Size of advert from a selection.i.e Half Page, Quarter Page, etc...Magazine to run in, choice of 6, Months advert to run i.e. October, November, Fee's charged for advert linked to size of advert, signed by docu-sign or similar. Submitted and pay via paypal or visa etc...
Desirable solution would work with either Sage CRM or Sugar CRM.
I can provide document / paper example of current paper based system.
A sample of current paper order form is below. I think the idea that the app means the sales guy can download and demo it to the customer is a great way of empowering the customer to re-advertise through the app at a later date.
It looks like a simple app in the first instance to take advertising bookings and payment. Future developments could be to have advertising features scroll through it from customers in the magazine.
