We are looking for a very simple app that does 1 basic function. Submission of a form with an image attachment. This is for a pharmacy which needs customers to submit their prescription for delivery. This form has a few fields like Name, address, number, email ect, with an attachment feature which can grab photos from gallery and also can take photo from camera and sends to an email address. Possible extra feature could be saving of the submitted prescription on each device for later access for a refill. This can be extra.
