Build out a Web Form with Reporting Functionality

High Level Overview:
• We currently have two projects that have reporting metrics and release data that would benefit from having everything in a DB.
• The primary focus on data is to capture PMO and Testing Metrics.
• Currently the teams are using are using MS Excel to track most of the data.
• The objective is to create series of web forms for data collection as well as detailed reporting.
Where we are currently:
• MS SQL Server has been created to try and begin to map the data inputs.
• There are some data conversions that are taking data from Excel documents and migrating them directly to the DB.
• The web server that is currently being used is part of AWS (Amazon Web Services).
What we need moving forward:
• We have approx 12-15 reports that we need to build out on the web in total. This first project is just 1 report for output and the associated web form to input data.
• For most of these reports we need to be able to not only review output reports, but also the ability to enter in data, and make changes.
• Most of the outputs are just grids of data being dumped from the DB.
• We will need the ability to also generate bar and pie charts based on some of the metrics.
I have attached a high level sample of what we are looking for for this first report. The document will show the Edit screen view and the Report View, the only item that will be needed that is not in the actual attachment is the Add a new items screen but the form fields for that screen should be almost exactly like the other screens.
We have about 12-15 of these reports that we would like to look at doing, and this report is just one of that group. If we find the right candidate we would like to then farm out all of the rest of the reports to that person.
We would like to turn this project around quickly! If possible we would like to complete the project in approx 3 days full time. Again we have approx 12-15 other reports.
