Custom Module to Create Drupal 6 Custom Forms

Please create a custom Drupal 6 module that will implement 2 data entry forms and 2 report forms. This project requires that you know PHP 5.2 and that you understand Drupal 6 modules and internals.
A very detailed requirements document is included in the bid request package (.zip file) attached to this bid request. Here is a summary of what we need:
1. Create code to manage a data entry form (create, modify, delete) for refunds. Create another form to show refund data in a tabular form (report).
2. Create code to manage a data entry form (create, modify, delete) for returns. Create another form to show return data in a tabular form (report).
3. We provide the 'create table' SQL scripts for the returns & refunds tables which your code will use for storing/retrieving data.
4. Please do not use CCK/Views to create these forms and reports.
