I have (1) a simple (only 2 fields) opt-in web form created on Aweber's (autoresponder) site. It works perfectly, even when hosted on an external site. However, it just does not look very creative. I also have (2) an opt-in web form template I purchased (I have yet to replace any text.) which looks great. Finally, I have (3) a list of instructions from Aweber on how to modify the html on a third-party web form that was not created on Aweber's site so it can send the relavent data to my account on Aweber.
I followed Aweber's instructions as best I could, but I cannot get my opt-in web form template, hosted on an external site, to send the typed in data to my Aweber account. I keep getting an error message from Aweber saying, Sorry, the account you tried to access was not found in our customer records. I thought that if I gave these 3 items to someone with a lot of experience with web forms, they could modify the third party template so it would send typed in data properly to my account with Aweber.
Can anyone help? Sorry, but not much of a budget.

An html file of my great-looking opt-in web form template with the coding modified so that when information is typed into the fields (only 2 fields), it will send the data to my Aweber account (exactly as the not-so-attractive form created on the Aweber site does.)
Also, a brief non-technical explanation of what I did wrong on my attempt.
