Basic Product Catalog

Integrate a basic product catalog (less than 15 items) into an HTML site. This would preferrably not require MsSql as the client's hardware will not allow for this. If MySql is needed, we will get hosting set up. Calculations include the total cost per item (number items x cost) and total cost for the order. Must have a secured connection to send credit card info over and then the client wants to type in the card info into their terminal for now. First time posting here but can provide better requirements. I hope to find some resources here to work with in the future - please mention what other skills you have as I have other projects in the pipeline as well. Attached is the paper version of the form we need to recreate online.
