I want to (2) major changes to an existing BANNER.

I am attaching the FLASH file with the bid... To win the bid you must supply demos or examples of previous work.

The website is created for two different types of users...

1. DOCTORS

2. PATIENTS

I would like the banner to be able to guide whichever user to the correct part of the website.

EXAMPLE:[

I would need only need 3 buttons

1. Home

2. EYE CARE PROFESSIONAL

3. PATIENT

I want to add a search box.... If you cant do the MYSQL functionality that is ok, I can have someone else take care of it for me...

but if you can... I would like a search box to search the lenses and manufacturers found in the table listed on this link <[url removed, login to view]>

An example of what I am looking for could be found here <[url removed, login to view]>

This search box would have to search my MYSQL database and I want the pop-up windw that the [url removed, login to view] site shows

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
