Build a provider search feature for our Website

We need a script to allow insurance members to search for dental providers in their area. We can build the front end - web page to capture location (zip code or city), provider type (or all), language, and a field to search by name (of provider or company). We will have frequent updates to the provider data which is available in csv and text file formats. The script must be able to apply logic to query the database and return the results. For example, if the location is in Arizona, it would return general providers within a 30 mile radius and specialty providers within a 60 mile radius. If it is in California, it would return general providers within a 25 mile radius and specialty providers within a 45 mile radius. So each state regulates specific parameters for search radius. Search results should be hyperlinked to pull up contact information and location on Google maps. Our current website is done in Dreamweaver. This script can be written in any program that is current and appropriate. Must be completed by October 1st for testing. Live by November 1st for use.
