Create a v.v simple page in Google Apps that collects Emails entered

Hello,

I am thinking this can be done in Google Apps or IFTT.

I want to design and make a page that will :-

a) take 3 email addresses that anyone enters into is and stores them in a format I can then import into mailchimp. Also their name and recipients name fields.

and then :-

b) it sends you to to another page (a bandcamp page to download an mp3 but it's not so important).

It needs to look very simple (I have a simple graphic to provide)

So basically the whole point of the page is to get our bands fans to send our an email to 3 of their friends asking them do check out our music, and in return they get access to a free song when they have entered the 3 email addresses.

We deed to make it very very easy for people to enter those email addresses to us.

Could use Google Apps or IFTT to automate it without worrying about web hosting.

THE PAGE DOES NOT NEED TO BE FOOL PROOF AND SECURE. This is not important at all. I am not concerned if people can 'trick' the page and get access to the next page without entering their emails. They will not do this and I trust them. It is just to make it easy to enter 3 emails on a good looking page.
