Contact form that saves on MySQL database

Hello, freelancers! I have a contact form, and I need a really simple system that saves submissions on MySQL database. I want to be able to change the number of fields in this form without changing anything on DB nor PHP code, so it should (when submited) recognize if all fields are in table and if any is not there, it should create this column by itself. For now, the contact looks like this: [login to view URL] I will provide HTML form, and there is no need to make any validator script (none of the fields are mandatory). One last thing demanded is a really really simple page with password to allow me download a XLS (or CSV) with information recorded in DB from this form (all information from all columns).
