Auto Loan Application with validation and email

This project is for an auto dealer sales promotion company. They advertise auto loans for everyone (bad credit, good credit, etc) through special sales events. They need a good looking online auto credit application form. The form will collect the relevant information from the visitor and will then Email a nicely formatted (using alot of CSS), printable form so the dealer can print it out and have the visitor sign it. The emailed form will look very much like a professionally printed version. Why email? We are going to use a email to fax gateway to deliver the nicely formatted form to the dealer's fax machine. Here are some examples of sites that already have this kind of thing. We are looking for a nice, attractive site that is NOT a copy of these, but similar in quality. The visitor should feel they are doing business with a reputable firm. The good design will help to leave this impression. Some (but NOT all) of the fields in the form will be required and most be completed before the person can submit the form. Notice that these require you to enter a zip code. We do NOT need this feature. We just need the form itself. But we do want the form to be visually attractive through the use of CSS. Should probably have it log the applications somehow in case the email delivery stops working. That way the forms can still be printed from any browser. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Hosted on Linux platform. Can use PHP or Perl to power the backend of the form. Do NOT bid ASP. This is being hosted on a Redhat Linux server.
