Adobe Acrobat Form Processing

Create two fillable PDF forms that submit to an email address. Forms will be posted on the website and give the user the option to download and fill out and post from their? desktop? if desired. ## Deliverables We would like to model the forms on this site: <[login to view URL]> Under the forms link at the bottom of the sidebar menu, we need identical fillable forms that will submit to an email address hosted by our website. contact@[login to view URL] We'd like two forms with the same? form fields as Dr Shafei has on her forms. Both forms? will be linked? on the sidebar of our website [login to view URL] Dr Shafei has given us permission to use her form layout, so we just need the same form structure set up with our logo added to the top left of the form and have it submit to contact@[login to view URL] Attached is my attempt if its a helpful or saves a bit of time.
