Appointment System

I am looking to get a personalized appointment setting system added to my website. It is a soccer training website and I want people to be able to access the website and set up an appointment. (maybe it could force a refundable fee for making the appointment that way if they fail to show up we keep the 5$ but if they show up the $5 get deducted from their overall payment, or they get their money back and owe us in full at the appointment.) It should show time of availability and not, just as a normal program would and it should be user friendly and easily accessed off of our existing website. We also do individual and team training. Individual training is 1 hour and team training is 1.5 hours. So an option should be available to select if you want individual or team training and depending on which option shows the length of time and where it can fit in.
