Contact Box

Project - Contact Box I need a contact box coded that uses sendmail (php) to get sent to 2 email addresses. The box should have text that reads (below in Verdana, size 1 font): 'If you would like more information about [login to view URL], please insert your email address' It also should ask where they are referred from (and have them enter info). The information should be collected, and parsed into a form and sent to 2 email addresses, disclosed upon selection of coder. The box should have the same look as the right column of [login to view URL] homepage wiht the 'news boxes'. i want the box for our site to look like the ones on yahoo and to be layed out great.
