Create 100 Forms in Online Survey System

I have about 100 different forms that each have between 1 and 3 pages (overwhelming number has 2 pages).? The average survey has 15 different questions of various types (free text, drop down choice, rank, etc.).?
For your planning, our estimate is that it takes about 30 minutes per survey once you get started (we've done about 20 similar ones already).? Additionally, it is helpful to know VERY BASIC HTML codes since it helps in formatting and presentation.? We can also direct you to an online HTML editor that we find works well.
Not all PDF forms translate exactly into our survey system, so sometimes you have to be a little creative about how to accomplish the goal best, but most of it is pretty straightforward.? The survey system permits "branching logic" so we would expect to use it where it makes sense.
I will provide for you all the forms in PDFs.? You have to put it into our online survey system, which is very similar to surveymonkey.com.
I am setting deadline at one week, but it is expected that you would start delivering surveys as the week progresses.
