acrobat form for offline and online purposes

We have a scanned form which we normally fill in with a pen and print out. The form has 4 duplicate pages which are the same but printed 4 times, as a copy for different sources. We would like to scan this form and use it as a underlayment of a real pdf form so we do not have to write on it again, but via the computer fill in certain fields and print it out.
Also this form need to be coupled to a MS access DB where addresses are stored which we need to use in a selection box.
Also the form need to be placed on a website for online purposes, but also offline on the pc for printing.
It is a quite easy project, for people who have skills creating forms with acrobat professional.
