A4 Photoshop form needs editing

I need an A4 sized page order form template completed and checked.
I'm not a Photoshop expert so I've done as much as I can. Now I need some one to fill in the blanks and check the spacing and margins.
What I need:
*Light grey underlines under personal and work detail entries.
*Postcode boxes to be redone symmetrically.
*Copy product boxes to the bottom of the page and make sure box borders are all the same color and have the same margins
*Check all margins and information is symmetrical.
Also, I put some underlines under "Hazet Items" which need to be deleted.
