We require a web site to be developed for the purpose of registering students from multiple schools for an event. The site will require some basic information about each student (detailed below), that will be entered by an administrator at each school. We will be providing a list of all the schools, and an email address for an administrator at each school. Using this list, we need to send each administrator a unique code that will be used to access this site, and use the site to enter the names of the registrants. The registration form will have the following: - Top of form will have a summary of the student criteria to register for Mighty Peace Day (Text that we will provide) - Drop down menu for the list of schools that the primary contact person can easily select which school they are registering students for - Field for Primary Contact – this will be the teacher or employee that will be overseeing the students for that school. We will require the primary contacts name, email and contact number. - Field where the students names can be input (first and last name only). Drop down menu for food option beside each student’s name (selection and price of each item and option to bring own lunch) * beside each person being registered. There will be an optional blank field for special needs, such as mobility for students in wheelchairs. - Fields where additional teachers and chaperones can be listed, including phone numbers and email addresses. Administrators will need the ability to come back and edit that list at a later date, should they desire to, adding or removing any information.
