Create a desktop application which connects to a MySql database to store and retrieve data

The application will run in a windows environment. It will have a login screen. Will allow user to enter three sets of data; vehicle type, registration number and color There will be two buttons on the interface; Generate and Next The generate button will send the data straight to the printer The next button just clears the form for the next entry The generate button will do two things: 1. save the record to the database and also 2. print the fitness certificate I have also attached an image to show the printed certificate The printed certificate will have: 1. Vehicle registration number 2. Vehicle type (MC, MCY, MV, ML, MB, MIB, MT) 3. Expiration date (the program can grab the system date and add 1 year to the current) 4. Fee paid for the certificate (the fee could be tied to the vehicle type eg MC fee = $5) 5. Certifying officer (the person logged in) 6. Date if issue of certificate Please see attached images.
