Create VB Form that fills in a Word Template

For this project I will need a VB Form created with standard bookmark inputs for the form selections. It will be one large form which will fill out the Word Template. It's pretty straight forward. I have all of the options for each field ( radio, check, text, date, list, etc.) and where they need to go, I just need the programmer to finish it off.
Brief description:
Input will be data from a form and a MS Word Template.
The output will be a document based on the information which was entered.
1. User will fill out a form with information such as names, dates, airports, times, locations, and a few other bits of info.
3. The application will use the user input and the template to create a letter document.
I have attached some screenshots of what i am looking for. Please keep in mind that the form is very rough but gets the point across well.
This will be for Word 2010. I can send you more details if you need them to bid properly.
