Access Database for tracking inquiries

We are looking to have a simple access database made that tracks the following: • Phone calls and walk-ins per day basis • Inquiries on a per week Basis • How did you hear of us on a per week basis There will have to be a form created that the user can input data (See Tracking Sheet for Data to be inputted (PDF)). The inquiries and How did you hear of us selections would have to able to be added to as we come up with new selections in the future. We will need graphs/charts to be created based on the database data as per example specified in the pdf supplied. We will need to be able to select time periods to create the chart from date pickers. Date ranges could be by week up to per year. We will need to be able to print charts/graphs. If possible it would be nice to have some of the charts overlay each other (Inquires overtop/beside How did you hear of us, to compare) User interface has to be user friendly and easy to navigate/input data. Any other suggestions or recommendations would be welcomed. PDF NOTES: Page 2 is sample Tracking Sheet that is used currently. This is the basis of all entries. The third and forth page is SAMPLE DATA ONLY and how the chart could look.
