Convert a Template to a Website

Hi,

One of my clients need a web site. I have the template and logo with me. I want a designer to make the template as its is for the company providing services like Web Development, Web Hosting & SEO. The template is responsive and single page, with lots of elements. Max elements should be used. A contact us form is also required. Rates of the hosting plan should be displayed also(Pricing Table Element Included). The work should be completed with in 4 days. It should be a completed work with all the datas inserted so a designer with content writing skill will be given more preference. Details about the company for about us section will be given, rest of the content should be added by the designer, required details can be asked.
