Convert a Template to a Website

I need online site/tool to keep track of number of admission done in hospital during different shifts by doctors. Background, I manage hospitalist physician group who does admission during 3 different shifts (8am-5pm, 12pm-10pm, 8:30pm to 8:30am) I need to keep track of these admission numbers i.e. how many admissions in which shift happened, for four different hospitals, and analyze this numbers to see which hospital has more admission for which shifts. That way I can keep up with staffing of physician. Data will be classified in 6 -8 (one liner) questions like Date: xx/xx/xx 1. Admissions done by ED admitter - 2. Admissions done by swing shift 3. Admissions done by night shift 4. Hold over when night shift arrives 5. hold over left by night shifts 6. holdover admissions when swing shift arrives 7. pending transfers Requirements, 1) I have to be able to have physician's enter the numerical data on website from different hospitals 2) I have to be able to see each day's data separate and also be able to create graphs for different date ranges for particular group (group 1-7, as written above) to analyze the need of physician staffing easily without too much knowlage of microsoft excel... as this will be used by multiple people.. 3) All four hospitals will have their separate data collection link/sublink or site 4) keep this data collection for years 5) would like to have some example of work you or your company has done.. Available resources 1) reasonable finance to make this happen in short period of time 2) hospital's website to possiblally upload this tool vs link, if possible OR if can not upload in hospital's site then i am ok with separately hosted site Thank you,
