Convert html php form to Dreamweaver - repost - Repost - open to bidding

The attached 2 forms are in HTML. I need the converted to DREAMWEAVER TEMPLATEs I do not need the material outside the green borders, only the form and the formaction. The traffic [login to view URL] form is a simple form. The FInd me a lawyer form [login to view URL] sends the usual form email to us, but it also sends a copy to our mysql database. If you copy the existing php correctly that should not cause any problems for you. This form is also in the secure HTTPS area since the information the client sends us is private I want to see a demo of each form working so post it on your web site and contact me when it is ready I want these forms done without delay, A day should be plenty of time for both The forms are added to this project. DO not post bids unless you can do this project. No boilerplate spam messages listing your projects. This is a simple job and I could do it myself but I am very busy these days.
