Create custom 3 page Microsoft Word/Adobe PDF form from an existing document

I need to have a custom Microsoft Word form created that will be three pages long. It's a form to document information necessary for a data migration project. I have all the fields on an existing form, but is ugly compared to what I know can be done by an expert. I would also like the form turned into an Adobe PDF form so that it can be electronically filled in by clients.
I would like to start with the form that I currently have and have some layout applied to it so that it looks professional (a good example is Microsoft Word's "Patient Registration" Form. I am currently still defining the form and adding a third page so the attached is an example of the fields. I will need to have several revisions before acceptance in MS Word format. Completed project should include both Word document and PDF fill-in form.
Some clarifications - I need to first create a custom Microsoft Word document from my (ugly) form. Once we have the form correct and approved I would like to then turn that into a PDF form. I can go a little beyond the established budget - that's a limitation of Freelancer.com, that its not a $200 USD project but I recognize that its more than a $30 project. I need something that is good and it might take 2-3 changes. I have a week or two so its not a huge rush.
