Gala Corporate Invitations

I need a simple but elegant corporate invitation designed for our Children's Gala. It will be a fold-over invitation, 5x5(inches) with our logo on the front and the inside will have information and another small logo. I also need a reply slip with their answer on it (very simular to a wedding reply card). I will need all the raw files handed over and also presented in the format ready for the printers to print without edit. The layout must be exact. Employer will receive exclusive and complete copyrights to all work purchased.
