I need an online database to be used as a personal logbook and also to be used for producing reports at the end of each month.
I have the Microsoft access database with all the tables and fields. I do not know how to form the right relationships between the tables so that they work online. I also am not familiar with the lookup fields and how to use selected predefined data in the fields.
I need simple front ends for the site to allow adding, modifying and deleting data. The scripts linking to the database should be ready to go on a windows server. Prefer Microsoft access database so that I can analyse data offline.
The structure of the database has the following tables
1. Table : Patients details
Fields: Auto ID number, Hosp number, Name, Date of birth, address, Email address.
2. Table : Symptoms
Fields : question 1 (1 answer out of 5 choices) question 2 (1 answer out of 5 choices) etc etc up to question 5, followed by a field for comments

Fields : Category, Diagnosis (A general diagnosis will be chosen out of a list of about 20), Sub diagnosis, further subdiagnosis. Comorbidity, A comments field. A script should be used to narrow down the list of options when a category and diagnosis is chosen.
4. Table : Operation Details.
Fields : Hospital name, Consultant name, Surgeons name, Assistants name, Anaesthetists name, Op start time, op finish time. Elective or emergency, Approach (Anterior, Posterior, Combined, Laparoscopic), Spinal Cord monitoring (Yes or No). Drains (1,2 or 3). Blood loss, Bone graft (rib, pelvic, vertebra or substitute), Research specimens (Yes or No). Intra-op complications (free text)
5. Table : Postop Details
Fields: Length of stay, Orthotics( Brace, shoe raise, crutches, walking stick or wheelchair), complications (choice of about 12) Xray findings. Follow up (Number of weeks ?)
6. Follow up Table : Symptoms
Fields : question 1 (1 answer out of 5 choices) question 2 (1 answer out of 5 choices) etc etc up to question 5, followed by following fields; How many weeks since op, comments
The database should allow different users to log on using own password. Only administrator can add to patients table ie add, delete or modify new patients. Users can only add or modify other tables. Patient details table must exist before other tables can be formed.
The following relationships should apply online;
Each patient can have multiple diagnoses. Each diagnosis can have multiple operations. Each opeartion can only have one postoperative details table. Each patient can have multiple follow ups.
