Reverse engineer old PCB

I have an old uncomplicated PCB that is probably some 10-12 years old. It is my work. There is no IP rights on it. It was a garage project done many years ago which I simply shelved and subsequently lost the notes. It was encased in a casing and is still clean, in good shape and I've simply touched it up lately to ensure it still works.
But I now need:
1) a schematic diagram done and description of parts,
2) the software for the small microchip so I can program and set parameters.
It is really very simple.
