Help with cover design for my upcoming novel

Hello everyone! So, I'm getting ready to self-publish my third book, but I'm struggling with the cover on this one, and I'm hoping someone can help design a cover for me that I'm actually satisfied with. So far I've already paid someone to design one for me once, but later, after some changes were made to the story, I decided it was too serious and didn't really fit the tone of the book. Then I had put together a few new ones myself, but have issues with all of them. I have attached all of these covers for your consideration. I like the idea of having one where it's a couple embracing, even just their legs, and I like the B&W one a lot and the public response to it was very positive, but some complained about the woman's legs and outfit, claiming she looked too young. But now I don't know if that is what I want to go with at all. Basically, the book is a collection of (old-fashioned) romantic short stories, but they are also very simple slice of life stories. But they all connect and revolve around one particular story about a man who is in the hospital preparing for brain surgery and he befriends one of the nurses there. So I started thinking maybe a cover with a heart/blood pressure monitor with a heart in there somewhere might be good. Something hospital room related but still implying romance somehow. I'm not dead set on using illustration or a stock photo yet either, I'm open to both. I just want to explore ideas. I have also attached a word doc my working synopsis along with all the covers I've almost used. Sorry for all the rambling. I'm really excited to get this book published, now I just need to get excited about a good cover for it as well! Let me know if you think you can help. Also, if you have knowledge of the size and formatting requirements for self publishing sites such as Amazon's create space and KDP (for digital versions) that is definitely a bonus. Thank you!
