pdf template for acupuncture file

I'm acupuncturist (I work in french. I'm from Montreal, Québec) I need to create my personnal pdf Template (in french) to upgrade my speed to take live note in front of my patient and write the treatement quickly the care that I give. I have the software to run it. What I need is to create my pdf Template. I'll use a stylet (computer pen) to write... no keaboard or less as possible... I want to use acupuncture chart. I want to be able to circle the acupuncture point that i'll do during a treatement on an acupuncture chart with all the 14 meridians instead of writing it. Front and back side . unisexe I need it to be precise as possible, clean and easy to read with the point numbers. All that I found on the net is unusable, to much information, or incomplete... If you start with what is on the net, it can be good. But I need it to be minimalist, clean and clear to read. I'll write information directly on the chart I'll upload a picture of a draw. On the paper I need a place to take note/evolution and a place to write down what I do during the treatement. 1/3 for note and 2/3 for acupuncture chart and I have my logo (Clinique DO) that can be discritly place in a corner somewhere with my name : Stefan Pelechaty, Ac A-020-08 It will evoluate with usage but i'll start with that. hope to be clear enough for you thank you :)
