PROJECT: [login to view URL] - PQi360 Professionalism Development System - Phase I
SUMMARY: Replace the current Google Forms and Google Sheets version of the PQ-i with a more accessible, scalable, maintainable, reliable..etc. longer-term solution. Create a simple web-based input form (similar to the Google Forms design) which replaces the (2) Google Forms for assessment input and differentiates between a Self and 360 assessment. Create a simple web-based edit form (similar to the Google Forms design) (ie...like an Admin Panel) which replaces the (2) Google Forms for editing the assessment for the Self and 360 users. This can only be accessed by an administrator and could be a simple icon on that assessment form which brings you to an Admin login and admin page to manage the google form on the back-end. Migrate the Access RDBMS to either mySQL or PostegreSQL utilizing the tables, queries and logic built into Access/Excel as a guide. Preferred infrastructure technology is L.A.M.P. on the latest supported versions (see below). Preferred software architecture methodology is HMVC/MVC or similar. We would like a simple interface that replaces our current google platform. We would like simplicity at first so we can migrate our users over to it and then add more enhanced features in subsequent versions.
NOTE: We are using two simple Google Forms surveys with a Google Sheet behind each form for data capture then we are pulling the data from Google Sheets into an Access database. We want to eliminate all of this and move it to simple web-based technologies.

Preferred L.A.M.P. Technologies:
Linux: Ubuntu 14.04 LTS (will be installed on server)
Apache: Apache 2.4.12 (will be installed on server)
DB: mySQL or Postgresql (developer determined)
Programming: Perl, Python and/or PHP (developer determined)
I have development servers available with Ubuntu 14.04 LTS and I can provide access once you are ready to test it in my environment before deployment.
