Re-create Google Forms with Jquery Mobile Form Elements

I would like someone to re-create a Google Form using Jquery Mobile with some dynamic select menus based on values stored within Google Sheet.
You will need experience in Google App Scripts, Javascript with Google forms and Google Sheets.
You will need to demonstrate the form on your own service.
Full payment will be made upon supply of code and confirmed tested ok.
The form field are as follows:
Name - Select Menu values from Google Sheet "Names" Column A
Batch ID - Select Menu values of a short list of Active Numbers from Google Sheet "Batch Active" Column A & B
Date - Pre-set with current date on form load, can be changed with Popup date picker.
Time - Slider with minimum time as 12:00AM maximum 11:59PM. Slider values in increments of 15mins. Pre-set to the nearest increment based on current time (e.g current time 12:20PM slider Pre-set to 12:15PM)
Vessel ID - Select Menu - Pre-set with the Vessel ID from a Google Sheet "Last Vessel" using the Batch ID as reference selected at beginning of form. Menu values from Google Sheet "Vessels" Column A
Temperature - Slider with minimum value 10 maximum 130. Slider values in increments of one decimal place (10,10.1,10.2,10.3 etc)
Pressure - Slider with minimum value 25 maximum 35. Slider values in increments of one (25,26,27 etc)
pH - Slider with minimum value 2 maximum 9. Slider values in increments of one decimal place (2,2.1,2.2,2.3 etc)
Notes - Standard Text Box
As per standard Google Forms data on submission stored in Google Sheet "Forms Response 1"
Attached are screenshots examples of a basic Google Form and what a Jquery Form would look like based on the fields. Also attached is an export from a Google Sheet.
This is one of many forms we would like re-created, this project is just for one form in an attempt to find a freelancer who is skilled to deliver.
