[login to view URL] form from ajax

Hi!

I'm using [url removed, login to view] to validate my forms. My form html code generate in php and get with ajax call. Form html placed to bootstrap modal.

I have mandatoryIcon addon and when I open first time modal with form, i get error "[url removed, login to view] Uncaught TypeError: Cannot read property 'notEmpty' of undefined", next time error not displayed. Why?
