javascript_form_component

I'm looking for JavaScript expert to build a special component which is to be used by other programmers. Background: User has a lot of paper forms to fill in. Form's questions has answer options Selected or Not Select. It is not possible to ask users to convert these forms into HTML form because they need to print the selections on the paper form directly. Component requirement: 1) The component should have 2 modes: 1) Design mode and 2) Input mode. On Design mode, user will scan paper form to image. The component should allow uploading the image as the background for design purpose. 2) On design mode, user are free to add checkbox on any position on top of the image. When adding new checkbox, the component allow user to assign one property "item_code" to the the checkbox. 3) The component store the background image, absolute position of each checkbox relative to the background image and "item_code" values into a propriety design file. 4) programmer can call the component by JavaScript function to load the design file in input mode. A callback function will be provided when calling the function. 5) In input mode, the component should display the image as background and show checkbox on the exact position relative to the image. 6) User is allowed to click on the checkbox to indicate selected the "item_code". When user finished his selection and click a finish button, the component should turn the selected checkbox into a "x" mark and merge the background image with the "x" marks into a new image. 7) The component return selected "item_code" to programmer's callback function and also return a url to retrieve the merged image file just created. Please use JavaScript to finished the job as much as possible. We have tomcat server, redis and mysql at the back-end.
